    Manager, Palliative Care Outreach – London (Temporary Full Time)

     

    The Opportunity: 

     

    At this time, the South West Local Health Integration Network is seeking one Manager, Palliative Care Outreach (“Manager”) to drive outcome improvements for people with life-limiting diseases in the South West. In collaboration with health system partners, the Manager formulates and executes a coordinated strategy for the delivery of Hospice Palliative Care based on leading practice throughout the South West Region.

     

    This is a temporary full time opportunity for a duration of approximately 12 months.

     

    What will you do?

     

    As the Manager, Palliative Care Outreach, you will: 

    • Participate in the formulation of implementation planning for the Hospice Palliative Care Outreach Program, including yearly priorities for action and yearly regional targets.
    • Establish and maintain the Hospice Palliative Care Outreach Program partnerships necessary to advance improvements in Hospice Palliative Care outcomes.
    • In consultation with the Governance and Steering committees, manage the clinical quality improvement agenda for the integrated Hospice Palliative Care Outreach Program and will review program indicators on a regular basis with Director, Home and Community Care and Senior Leadership. 
    • Hold regular (at least quarterly) Regional Hospice Palliative Care Outreach Program Steering Committee meetings.
    • Establish and maintain excellent communication with partners with respect to the Hospice Palliative Care Outreach Program, which includes yearly priorities, proposed funding allocations within budget, performance targets and program performance.
    • Work collaboratively with the South West health system partners to facilitate Hospice Palliative Care service enhancements and to set targets for the South West Hospice Palliative Care Outreach Program.
    • Establish and implement quality and improvement plans for areas identified through program reviews.
    • Consult regularly with leaders and system partner organizations on plans for quality improvement and yearly targets for improvement.
    • Provide ongoing leadership to the Hospice Palliative Care Outreach Program team for the development and implementation of an integrated Hospice Palliative Care program in London/Middlesex.
    • Develop, implement, monitor and evaluate area operational plans, goals and achievements.
    • Identify budget requirements and allocate approved funds in accordance with the Hospice Palliative Care Outreach Program priorities; monitor, evaluate and manage budget. 

    Location: This position is located within the LHIN’s London/Middlesex location.

     

    What must you have?

     

    Education: 

    • Post-secondary education in health science/administration, health promotion, or related field; or an equivalent combination of education and experience.  

    Experience:

    • 3 to 5 years of leadership/management experience with the ability to manage complex projects involving multiple agencies, budgets and stakeholders.
    • Experience and working knowledge related to service delivery systems across the continuum of care. 

    Knowledge, Skills, and Abilities:

    • A proven track record of effective working relationships with internal and external customers and clients and the ability to build teams and lead through system level change.
    • An ability to resolve conflicts and achieve consensus.
    • Excellent time management skills and ability to prioritize multiple, conflicting demands.
    • Superior organizational skills and solid knowledge of project management and quality improvement methodology. 

    Preferred Qualifications: 

    • Proficiency in a second language, particularly French.
    • An ambassador of workplace culture.
