Currently Recruiting (Regular Part Time) – the urgent team a portfolio within the ICT department will manage urgent referrals and calls to optimize patient experience, quality of care and patient safety.
The team will pilot a scheduling model consisting of a mix of eight (8) and twelve (12) hour shifts over the course of the weekdays/weekends totaling thirty five (35) hours per pay period.
As a valued member of our Home and Community Care team, you will facilitate the journey through Ontario’s health care system by assessing referred patients, determining their health care needs and eligibility, and ensuring they receive the services and care they need, where and when they need them.
