    Manager, Home and Community Care – Seaforth or Stratford (Temporary Full Time)


    The Opportunity: 


    At this time, the South West Local Health Integration Network is seeking one (1) temporary full time Manager, Home and Community Care to support a portfolio within Huron and Perth counties. The assignment is an approximately duration of four (4) months with possibility of extension.


    The Manager, Home and Community Care is responsible for managing daily operations to ensure patients and families receive effective and efficient services through the provision of care coordination services and long-term care home placement services.


    What Can I Expect To Do? 


    As the Manager, Home and Community Care, you will: 

    • Lead the delivery of Patient-Driven Care by building and maintaining relationships with community and hospital stakeholders, contracted service providers, and other health systems partners.
    • Ensure the effective delivery of patient care and operational support for a specialty area within the Huron/Perth region.
    • Supervise the performance of a Care Coordination team within the Huron/Perth area, including determining and assigning workload, and coaching and providing guidance on handling care coordination issues.
    • Analyze information from information systems/databases, service providers, patient reports, staff feedback, and industry trends and practices to formulate recommendations and provide input to planning processes for future programs, policies, and services.\
    • Be the first-level of management in a specialty area within the Huron/Perth region.
    • Resolve problems and conflicts regarding clinical and interpersonal issues that require consideration of multiple sources of information.

    Location: The Manager, Home and Community Care will primarily be located within the Huron/Perth region at one of the LHIN’s offices in Seaforth or Stratford.

    How Do I Qualify?



    • University Degree in Healthcare or Business Administration and/or equivalent education and experience.


    • Minimum three (3) years’ recent and relevant management experience in a unionized environment. 

    Knowledge, Skills, and Abilities: 

    • Knowledge of direct care coordination models used in community health care organizations and a good knowledge of community resources (e.g., services and programs), and roles of health care professionals.
    • Knowledge of South West LHIN business strategies, objectives, priorities and programs, and related patient care priorities and plans and knowledge of the evolving role of LHINs, and the issues and priorities within the healthcare sector and how these impact patient care delivery.
    • Effective communication skills to maintain a range of contacts with health professionals within the community and strong collaboration skills to participate on projects and committees with colleagues across the South West LHIN or local community agencies.
    • Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action and good coaching and communication skills to provide instructions and guidance to staff with respect to activities, challenges and questions.
    • Strong planning and organizing skills.
    • Efficient computer literacy in a Windows environment.
    • Regular travel throughout the South West LHIN boundaries and occasional travel outside the South West region. 

    Preferred Qualifications: 

    • Proficiency in a second language, particularly French; and,
    • An ambassador of workplace culture.
