  • Job Description

    The Team Assistant is responsible for providing administrative support and clerical assistance to patient care teams at the Mississauga Halton Local Health Integration Network (LHIN). The key duties of the Team Assistant involve acting as a link between Care Coordinators, patients and external stakeholders; providing patients and external stakeholders with information on the Mississauga Halton LHIN and other health and social services that is within the scope of the Team Assistant’s role, and entering data that is clear, accurate and meets corporate standards.

    Reports to:   Manager, Home & Community Care

    Category:   Temporary Part-time (to October 2019); 0.6 FTE (42 hours/2 week pay period)

    Schedule:   must be able to work afternoon shifts (1:00 PM to 9:00 PM) and weekends

    Team:   Access Care Team

    Current primary assigned location:   Mississauga Office, 2655 North Sheridan Way

     

    DUTIES & RESPONSIBILITIES

     

    Customer Service

    • Builds positive and effective relationships with internal and external stakeholders
    • Provides patients and external stakeholders with information on the Mississauga Halton LHIN and other health and social services that is within the scope of the Team Assistant’s role
    • Communicates with all members of the team to ensure information is distributed and obtained appropriately 

    Administrative Support

    • Directs referrals to the appropriate Care Coordinator
    • Prepares documents for Care Coordinator authorization, including requisitions, service/supply/ equipment orders, patient privacy letters, school letters and other templates
    • Orders, discharges or renews requests for supplies, services and equipment as authorized by Care Coordinators
    • Routes information between staff, service providers, patients, physicians, pharmacies and suppliers regarding patient’s service status, deliveries, discrepancies and delays in provision of supplies, services or equipment
    • Follow established criteria for the selection of service providers for supplies, services and equipment
    • Monitors need for routine service extensions and obtains authorization for continued service
    • Books/changes routine home visit, patient/family conferences, and reserves conference rooms as directed by Care Coordinators
    • Investigates and confirms suspended billing
    • Works under the direction of Care Coordinators to resolve billing discrepancies with providers
    • Directs Care Coordinators to inquiries about service orders and/or changes in billing from service providers
    • Initiates and monitors drug cards
    • Provides telephone and voicemail support for Care Coordinators
    • Accesses and updates team member calendars
    • Works collaboratively with Care Coordinators to maintain patient service calendars
    • Provides transcription services as needed
    • Processes cancellation of patient services upon discharge from Mississauga Halton LHIN
    • Arranges patient transportation as authorized by Care Coordinators
    • Maintains service wait lists
    • Maintains inventory of office supplies, forms and medical supplies

    Data Collection, Distribution and Accuracy

    • Enters patient data accurately into a range of documents and forms
    • Accesses patient information as required (e.g. discharge, on hold, rejected entries, corrections, admit/non admit, registration/intake, change of address/transfer)
    • Maintains an accurate filing system of confidential patient records
    • Follows established guidelines to assign new or transferred patient files to the appropriate staff
    • Prepares patient assessment packages
    • Obtains and confirms consent for referrals
    • Validates patient OHIP card numbers

    QUALIFICATIONS

     

    Education, Training & Experience

    • Completion of Secondary School required
    • Minimum two years of related experience
    • Administrative/secretarial experience preferred
    • Experience in the health care sector is preferred
    • Medical terminology training is preferred
    • Computer training is preferred
    • Experience with medical supplies is an asset
