Reporting to the Manager, Sub-Region Planning & Integration, the Planner, Sub-Region Planning & Integration is responsible for supporting the planning, development, and implementation of sub-region and locally integrated planning in accordance with the Mississauga Halton LHIN health equity mandate and philosophy of patient-centered care.
DUTIES & RESPONSIBILITIES
- Supports the development and implementation of local planning with a focus on integration, coordination, service innovation and quality improvement at the sub-region/care community level in alignment with provincial and regional strategies.
- Partners and works closely with Clinical, Home and Community Care and Regional Programs portfolios and system partners to ensure alignment and linkages across sub-regions, regional programming and local planning.
- Supports and/or leads initiatives that ensure home and community care and primary care integration within care communities so that patients and families experience a cohesive system of care, and providers understand how their roles connect within the system.
- Ensures health equity and patient/family engagement lens guides health system and sub-region/care community planning and development.
- Facilitates engagement with health system providers, partners and local communities, including cross-sector partnerships outside traditional healthcare services (i.e. social determinants of health) in order to achieve key local deliverables.
- Works closely with decision support to understand datasets and analyze sub-regional health care system resources and performance to identify gaps and opportunities, initiating action to address provincial strategic priorities.
- Maintains awareness of current issues in the health care system related to the Integrated Health Service Plan strategic priorities and key result areas with a particular emphasis on sub-region/care community issues related to the implementation of change initiatives.
- Works collaboratively with key stakeholders and community partners to build local capacity and improve the system of care for patients with complex needs in Mississauga Halton region, expanding lessons learned for the purposes of population health planning and integrated planning and care delivery within specified neighbourhoods and care communities.
- Liaises with Health Quality Ontario and Ministry of Health, and Pan-LHIN leads for meetings, mandatory reporting, communities of practice and specific business cycle requirements for Health Links.
- Collaborates with communications and engagement team, and Health System Planning, Integration and Development Team to ensure alignment across sub-regions/region, and to integrate and align key messages and communication strategy in support of shared goals.
Other Related Activities
- Leads key provincial, regional and/or sub-region/care community initiatives with cross-functional teams, including tests of change and evaluation plans.
- Creates and controls all project management artifacts needed to effectively strategize, plan and execute regional and local initiatives.
- Develops, with key internal and external stakeholders and partners, the project work plan, monitors and controls the project plan.
- Supports the work of local steering committees and regional tables; facilitates team collaboration and drives work accordingly.
- Plans for and supports the implementation of business changes, plans, incentives and metrics needed for successful project implementation.
- Identifies, manages and gains approval of changes to the project definition and plan as needed; manages and resolves issues as they arise.
- Ensures that proper reviews and approvals are obtained for project documents, milestones and deliverables.
- Demonstrates hands-on management approach to ensure successful delivery of projects on time and within budget.
Education, Training & Experience
- University degree in Social Sciences, Planning; Health Care, Business Administration or related field; a Master’s degree is an asset (or equivalent combination of education and experience);
- Project Management Professional (PMP) designation an asset
- Five (5) to seven (7) years of experience at mid-senior levels at a social service, health care or related organization
- Experience and understanding of health equity and patient/family engagement, and patient-centred care
- Experience working with government and non-government funded organizations and understanding of LHIN’s mandate and their role in the broader system
- Exceptional project management skills to effectively lead a project team to achieve critical project milestones using a formal project management approach
- Demonstrated knowledge, experience and success in applying quality improvement methodologies is highly desirable
- Strong understanding of Mississauga Halton region, the health care system and community sector across the continuum of care, including social determinants of health
- Superior communication, interpersonal and facilitation skills to work with a variety of stakeholders and senior leaders across the health continuum
- Excellent negotiation, conflict resolution and consensus building skills
- Demonstrated ability to build and sustain excellent partner relationships and enable voluntary collaboration
