    JOB TITLE: Clinical Practice Education Specialist

    START DATE: August 2018

    DURATION: Full Time, Permanent

    REPORTS TO: Director, Professional Practice

    DEPARTMENT:  Home and Community Care

    LOCATION: Toronto Central LHIN




    This position will report to the Director of Professional Practice. This position will be closely mentored by the Client Services Clinical Practice Manager and is responsible for developing, planning, facilitating and evaluating education and development activities and programs to support performance excellence in the delivery of high-quality, evidenced-based, clinical care which will align with organizational goals and strategies.

    As a key “change agent” for Toronto Central LHIN, the Clinical Practice Education Specialist will work with Client Services and the Organizational Development team to implement interventions that facilitate organizational change and foster a learning culture throughout the organization.




    Learning Design, Development, Delivery, and Evaluation

    • Working closely with Client Services Professional Practice Leadership and the Learning and OD Specialists, undertake or assist in the development/sourcing, delivery/coordination of and evaluation of education and development activities to meet short and long term staff needs to fulfill strategic directions and plans of the organization
    • Contribute to the identification of learning needs based on information from a variety of sources, including formal needs assessments, staff feedback, performance feedback from Client Services Managers, etc., and assist in the development of annual learning plans within the Professional Practice portfolio as well as at the organizational level
    • Design, develop, and facilitate education and development programs using principles of adult education and tools that will enhance individual and team performance and supports evidence-based care in the community

    Change Management

    • Work closely with the Client Services Managers and Learning and OD Specialists to support change management initiatives regarding education, development, and orientation in order to achieve outcomes and build staff resiliency
    • Establish and maintain effective relationships with Client Services Managers, HR, and Communications to ensure successful implementation of change initiatives

    Clinical Consultation

    • Act as a point person and resource for clinical practice issues from internal staff and external stakeholders questioning clinical practice in the community
    • Participate in quality care reviews as appropriate to provide a clinical lens on the event and suggest recommendations for changes in care and practice
    • Provide feedback and participate in the review of new or revised clinical polices/guidelines/procedures
    • Be aware of changes in legislation by the various Colleges and determine their impact on practice at the LHIN and with our Service Providers

    Project Management

    • Provide support to Professional Practice and organizational projects impacting direct clinical practice and care coordination
    • Contribute to the development of project plans and follow established practices, timelines and budgets for the execution and delivery of initiatives
    • Performs other duties as required




    • Minimum of a Masters in Nursing and in good standing with the College of Nurses
    • Formal education and/or experience in adult learning and development
    • ET or Wound specialist certification would be an asset


    • Minimum 5 years of nursing experience. Experience in geriatrics and/or nursing in a community setting would be an asset
    • Knowledge of adult education principles, assessment and analysis of education needs and experience in education and design development (including the use of alternate education technologies such as web-based training) and evaluation is highly desired
    • Knowledge of change management resources, theories and practices
    • Superior interpersonal skills, verbal and written communication skills and excellent analytical and conceptual skills are required
    • Advanced computer skills and/or the ability to use a variety of software programs (e.g. MS Office, Storyline, Captivate, Articulate, etc.)
    • Demonstrated ability to work independently and in a team, and to establish and maintain effective working relationships at every level of the organization and with external partners is required
    • Proven skills in project management, organization/time management, and ability to thrive in a rapidly changing environment
