    Rapid Response Nurse (Part-time)



    The Central Local Health Integration Network (LHIN), a Crown Agency of the Province of Ontario, is responsible for planning, funding, integrating health services and delivering home and community care services for over 1.8 million people living in northern Toronto, parts of Etobicoke, York Region and South Simcoe County. The LHIN provides over $2 billion in funding to more than 90 health service providers, including hospitals, long-term care homes, community health centres, mental health and addiction service providers and community support services.


    Reporting to the Senior Manager, Home and Community Care, your role is to ensure effective transitions from acute to home care for frail adults and seniors with complex needs and/or high risk characteristics, e.g. congestive heart failure.  The Rapid Response Nurse provides the first in-home nursing visit within 24 hours from hospital discharge for high needs seniors. Your responsibilities include effective communication and linkage with primary care; timely and effective rapid response nursing care. During this visit, the nurse will confirm the patient hospital discharge care plan, perform a thorough nursing assessment and clinical interventions as required, communicate the importance of primary care to avoid re-hospitalization, and perform medication reconciliation for the client.


    Scheduled hours and days require flexibility in order to meet the needs of the Central LHIN and its clients.  Rapid response nurses rotate through a 24/7 schedule which includes on-call shifts. Initial area and/or schedule may change in order to facilitate the needs of the Central LHIN in accordance with the ONA Collective Agreement.Work may also be required in different locations across the Central LHIN.



    As per the collective agreement.



    • Registered Nurse (BScN or diploma) and current registration with the College of Nurses of Ontario.
    • Emergency/critical care and /or Geriatric nursing certificate an asset.
    • Minimum of five years of direct recent and relevant nursing experience in a clinical role.
    • Excellent clinical assessment skills, i.e.: cardiorespiratory, hydration status, nutritional status, pain and symptom management. 
    • Excellent clinical nursing skills, i.e.: intravenous access, central line access, venipuncture, catheterizations, g-tube feedings, tracheostomy care, ostomy care, wound care, chest tubes, pain and symptom management, palliative care.
    • Effectively maintain a constant flow of verbal and written communication with others throughout the workplace as well as outside the organization. 
    • Able to communicate with clients’, their families, and other relevant individuals in order to follow through with care plan directives. 
    • Knowledge of the principles of adult health teaching.
    • Working knowledge of community resources and roles of health care professionals.
    • Solid knowledge of health care related legislation and practices.
    • Knowledge of direct care / case management models used in community health care organizations.
    • Knowledge of LHIN priorities, policies, practices and service standards.
    • Effective interpersonal and communication skills.
    • Effective organizational and planning skills.
    • Basic proficiency with computerized information systems.
    • Bilingualism (English/French) considered an asset.
    • Demonstrates commitment to the LHIN’s mission and values.
    • Demonstrated awareness of cultural diversity, as well as ability to behave discreetly and sensitively to confidential issues.
    • Must have a valid driver’s license and access to a vehicle.
    • Travel will be required.
    • On call duties required.


    This position's primary assigned site is Newmarket, Ontario, however there will be requirement for travel to other sites across the Central LHIN.
