    RAPID RESPONSE NURSE (Permanent Full-Time)


    The Opportunity


    As part of a system wide initiative to strengthen connections across the health continuum, the South West LHIN is directly hiring a permanent full time Rapid Response Nurse.


    The Rapid Response Program is an initiative to the LHINs to ensure effective transitions from acute to home and community care for two target populations: medically complex children and adults. This program will also ensure communication and linkage with primary care and provide timely and effective rapid response nursing in an individual’s place of residence.


    As a member of the Home and Community Care team, the Rapid Response Nurse will provide transitional support and care for patients from hospital to home, make connections with primary care partners, and perform essential medication reconciliation activities – reducing the risk of patients being readmitted to hospital.


    You will liaise with patients and their care team in reviewing discharge care plans, facilitating access to other health care practitioners/medical tests and assisting with medication protocols. You will also initiate care connections with a range of health care partners to ensure patients and their caregivers are well supported in their home and community.


    Location: This position is located in the South West LHIN’s London location.

    How Do I Qualify?


    • Regulated Health Professional with a Bachelor's Degree in Nursing.
    • Current, active registration or licence to practise in Ontario as a Registered Nurse (RN, BScN).


    Experience and Skills:

    • Minimum of two (2) years of direct clinical experience.
    • Minimum of five (5) years of relevant experience as a Registered Nurse.
    • Working knowledge of community resources and roles of health care professionals.
    • Knowledge of direct care/case management models used in community health care organizations.
    • Excellent interpersonal and communication skills to communicate with patients’, their families, and other relevant individuals in order to follow through with care plan directives.
    • Well-developed analytical and critical thinking skills.
    • Must have valid driver’s license and access to a vehicle.



    • Proficiency in French is an asset.
