Senior Manager, Short Stay 

Regular Full Time Position Available Patient Services - Short Stay

Location: Sheppard Site with travel across all sites

Reporting to the Director, Home and Community Care, this position is responsible for leadership, coordination and delivery of all client services within the Short Stay Program.  Key position functions include monitoring and responding to service information, analyzing and communicating system information for stakeholders: participating and leading projects that result in improvements to processes.



  • Significant experience in diverse and unionized community health care environments
  • Working knowledge of community resources and roles of health care professionals
  • Experience working in a variety of health care settings and roles
  • Degree in health related profession or equivalent experience
  • Certification in a regulated health care profession
  • Three (3) years’ managerial experience preferred
  • Good knowledge of LHIN priorities, policies, practices and service  standards
  • Good Knowledge and understanding of legislation governing health care activities
  • Knowledge of direct care/Care Coordination models used in community health care organizations 
  • Strong knowledge of short stay specialty populations
  • Knowledge of contract and performance management
  • Working knowledge of project management
  • Understanding of quality and process improvement concepts
  • Effective planning, organizing and evaluation skills to manage the delivery of quality service, provide information reports and take corrective action as required
  • Experience with scheduling Care Coordinators to ensure staffing is appropriately to meet the ongoing workflow
  • Adept in using a range of technological devices and applications including current client services applications and databases
  • Ability to document in various formats (email, client records, minutes, report
  • Effective communications skills to establish and maintain wide range of contacts within the organization
  • Experience with facilitating/presenting to large groups
  • Excellent oral and written communication skills to facilitate effective communication with all levels of LHIN staff
  • Strong collaboration skills to participate on projects and committees with colleagues across the LHIN, or local community partners
  • Ability to deliver information effectively in a variety of settings including one-on-one, team meetings, and presentations
  • Keeps direct reports informed of key issues and changes which may affect them
  • Advanced negotiation and conflict resolution skills
  • Coaching, mentoring and counseling skills
  • Ability to work independently and collaboratively with all staff
  • As the first-level of management in the area, position is required to resolve problems/conflicts regarding clinical and interpersonal issue
  • Uses information from multiple sources including client services to formulate recommendations and provide input to planning processes
  • Uses creativity/innovation to develop solutions to client service needs which are unique to the Short Stay population
  • Ability to fact-find using a variety of methods such as interviews, research, reports best practice guidelines and face-to-face meetings
  • Ability to identify and respond to high risk, complex situations in order to mitigate risk to the client, staff and organization
  • Position is accountable for ensuring direct reports function within legislative framework, LHIN goals, policies and procedures and within designated specialty best practices regarding Care Coordination and quality client care
  • Provides quality information and advice regarding functional specialty
  • Recruits, manages,  coaches, mentors and evaluates the performance of professional Care Coordination staff
  • Handles grievances at the first-line management level and attempts to resolve issues where possible
  • Facilitation skills to work with staff in developing input to strategic and operational planning processes
  • Provides advice and guidance to staff in handling non-routine situations and  Care Coordination issues
  • Support staff in identifying issues, testing and implementing change processes  to improve quality care and ensure service delivery to clients
