In alignment with the Mississauga Halton LHIN Integrated Health Services Plan and provincial direction, this individual will work collaboratively with the Vice President, Regional Programs, team members and health system providers and partners (both internal and external) to support the development of integrated, regional services to support the advancement and transformation of the long-term care home sector, in order to improve health outcomes for residents and their families, and enhance the experience of care and support a continuum of care for the most vulnerable seniors in the LHIN.
DUTIES & RESPONSIBILITIES
Function as a key contact with Long-Term Care homes within the Mississauga Halton LHIN and their corporate head offices on a regular basis building strong and supportive relationships as well as in-depth understanding of the existing programs and services in order to monitor and analyze progress of key initiatives, identify further development opportunities and needs, and identify issues and work collaboratively with the appropriate personnel to develop strategies to address any risks or challenges
Monitors Long-Term Care homes within the Mississauga Halton LHIN to ensure Ministry of Health and Long-Term Care legislation, regulations, standards and policies are met
Review, evaluate, analyze and follow up on any compliance issues brought forward by the Compliance Branch of the Ministry of Health and Long-Term Care
Act as a change agent to manage change and transformation in order to create a progressive health care system for the relevant program areas as well as the Mississauga Halton LHIN more broadly
Identify and supports program planning, development and quality improvement opportunities in the long-term care sector through a solid awareness and understanding of local population health needs, gaps, opportunities as well as innovative best practice strategies/approaches utilized in other jurisdictions.
In conjunction with Vice President and key stakeholders, provides expertise and facilitation for the advancement of a range of new and existing long-term care programs and services through the development, implementation, evaluation, ongoing monitoring of services across the continuum of care to support quality care for long-term care residents
Identify opportunities and proactively drive the development of innovative and best practice strategies within long-term care by facilitating active problem solving and solution development to formulate creative strategies with achievable program plans with key partners and senior leadership teams
Conduct analyses of the regional health care system based on data, community input and other sources for the purpose of identifying gaps/ opportunities
Establish planning and integration priorities that align with provincial strategies for seniors and facilitate the implementation of integration activities focused on acquiring greater efficiencies and effectiveness
Identify, advocate/support, justify and assist in the completion of business cases and funding proposals and allocations. Review and evaluate program proposals.
Work with Long-Term Care home operators, real-estate, development teams and the Ministry of Health and Long-Term Care to support the redevelopment of older Long-Term Care homes within the Mississauga Halton LHIN
Interpret, apply and work within the relevant health care legislation (i.e. Long-Term Care Home Act, LHSIA, etc. ) and identify opportunities for system changes based on any new legislate change
Participate and contribute knowledge and advice to various pan-LHIN and/or Ministry groups/committees at the direction of the Vice-President. Represent the Mississauga Halton LHIN at provincial program planning tables and ensure knowledge transfer to colleagues and partners within the LHIN
Education, Training & Experience
University Degree in Social Science, Nursing, Health Administration; a Master’s degree is an asset
Five (5) to seven (7) years related experience with program planning, integration and management, preferably in a healthcare environment; (or an equivalent combination of education and experience);
Experience and working knowledge of care delivery in long-term care, community and health care environments including understanding of long-term care, seniors and dementia care across the continuum
Solid experience in developing strategies, programs and/or initiatives to successfully address the needs of patients and families, through the various stages of care needs
Understanding of Ontario’s health-care system, key priorities and trends; particularly within the long-term care home sector.
Experience in program design development and evaluation, program implementation, health system planning and project management within a health care environment.
Innovative systems thinker with a passion for working collaboratively with key stakeholders
Proven skill and experience leading and implementing initiatives aimed at accelerating transformation and change management
Demonstrated ability to work with ambiguity and to take responsibility for developing and defining strategies, negotiating and facilitating complex sessions, and resolving knowledge gaps (individually and for the broader team) when working with areas that are not fully developed and require learning
Knowledge of tools, systems and databases used in patient care delivery and provincial tracking/monitoring, to help identify, analyze and support effective management, utilization, and development of outcomes
Excellent organizational, problem solving skills, time management skills that support the ability to rapidly shift focus and activities in response to changing priorities and manage multiple priorities and deadlinesProven strong, positive interpersonal skills to build and maintain strategic relations with diverse stakeholders.
