Director, Home and Community Care

Reporting to the Vice President, Home and Community Care, this Director is responsible for developing and leading the implementation of strategic plan related to the Home and Community Care portfolio.  As a strong healthcare leader you are required to have a high level of independence and is expected to provide leadership to ensure the objectives of the Central West LHIN are met; these include priority areas of leading the portfolio in the delivery of excellent care coordination, transition planning, quality home care delivery and strong collaborative partnerships that foster innovation in care delivery.

 

As an innovative leader you will support the commitment of Central West LHIN to improving patient experience by:

 

  • Providing quality care and ensure value for money to ensure care is available for patients with the greatest need;
  • Earning the trust of health system and community partners, to support patients across the continuum of care;
  • Working with primary care practitioners, hospitals, community agencies and others to coordinate care across the health care system; and
  • Fostering a workplace where people want to be, recognizing the strong correlation between staff satisfaction and patient experience.
     

You will have the responsibility for planning, organizing, directing and evaluating the activities of the assigned programs in Home and Community Care. As an accomplished leader you will provide leadership to direct reports and front-line staff to ensure exemplary experience for the patients and families we serve.

 

As an ideal candidate;

 

  • You bring a track record of success as a healthcare leader, with at least 8 - 10 years’ experience in health delivery system with at least 3 years at a senior management level with demonstrated experience in a leadership capacity.
  • You need to have a Master’s degree in a Regulated Health Profession or related discipline. Additionally, registrant in good standing with a Regulated Health Professional body, is preferred.
  • You are a strong collaborator to promote a systems approach that provides a seamless approach for patients.
  • You are a superior relationship builder with outstanding communication skills.
  • You possess demonstrated experience in effectively leading change.
  • You pride yourself of your business management skills, especially your financial acumen.
  • Finally, you have demonstrated experience in client-focused programs and services, and exposure to case management.
