Current primary assigned location: 700 Dorval Drive, Oakville
Reporting to the Director, Physician Engagement, Clinical Integration & Relationship Management, the Primary Care Advisor (PCA) is responsible for the engagement of Primary Care providers within Mississauga Halton Local Health Integration Network (LHIN). Key duties of the PCA will be foundational to enable the achievement of successful Primary Care Integration (PCI) across Mississauga Halton LHIN. The PCA will be assigned a catchment area that aligns with the Mississauga Halton sub region geographies and will communicate and promote programs, services, resources, and opportunities to Primary Care providers in accordance with the Mississauga Halton LHIN PCI Strategy. They will enable physicians to optimize resources and opportunities available to them to provide exceptional care, ultimately impacting system outcomes. The PCA will also support health service providers across the health care system in developing effective strategies for informing and engaging physicians about their services.
